Elevated Health and Community

Elevating individual health and building community, one event at a time!

Mission

Our mission is to disrupt health disparities and build a more human-centered healthcare system by strengthening patient advocacy, creating inclusive community health experiences, and equipping and cultivating equity-driven healthcare leaders and institutions.

Vision

We envision a future where high-quality, socially equitable healthcare is the standard—not a privilege. Our goal is to shift the system from a “sick care” model to one that prioritizes overall health and well-being, and every patient is empowered with the access, information, and resources needed to make informed decisions about their health.

What We Do

We bridge the gap between our communities and the healthcare systems built to support them:

  • Patient Advocacy: We provide direct support during appointments and care navigation, especially for those who do not have additional people to support them during their care journey and are impacted by chronic illness, systemic bias, or care disparities.

Health Literacy & Access: We partner with community, healthcare, and social support organizations to offer community-centered health engagements that provide the access, information, and resources community members need to take control of their health journey and make informed, high-quality decisions about their health.

Community Health Initiatives & Campaigns: We partner with patient-serving facilities and organizations to turn well-intentions into impact through the co-creation of culturally affirming and accessible community health engagements.
