COMMUNITY HEALTH WORKERS (CHWs)
Community Health Workers play a vital role in helping individuals manage their health and reduce the impact of health disparities. They provide education on healthy lifestyle choices, connect community members with healthcare services, and offer support and advocacy.
By working closely with individuals and communities, CHWs help improve overall health outcomes and promote wellness. As a resource to our communities, several CHWs have provided free Community Health Information Flyers.
